You may request to rescind your waiver and reinstate your Harvard Blue Cross Blue Shield coverage by completing a waiver rescind form. There is a $50 rescind processing fee for each request.
To Rescind Your Waiver
- You must have a qualifying event* to rescind
- Your request must be submitted within 30 days of the qualifying event
- You will be charged a $50 rescind processing fee
- Upon approval, your coverage will be retroactive to the start of the term for which you wish to rescind your waiver; coverage is not prorated.
- Insurance charges and the $50 rescind processing fee will be applied to your term bill
*Qualifying Events
Your waiver rescind form must be submitted within 30 calendar days of the qualifying event. You must include acceptable documentation of the qualifying event with your rescind form.
• Loss of alternative health insurance coverage (letter from your insurance company indicating end date of your insurance coverage)
• Birth of a child; legal adoption; legal guardianship (birth certificate or copy of legal guardianship/adoption documentation)
• Marriage or Qualified Same-Sex Domestic Partnership (QDP) (marriage license or certificate of QDP)
• Entry into the country (passport stamped with entry date)
