Submit your application(s) and print a copy of the confirmation page. If upon submitting your waiver application you do not immediately receive a confirmation page, contact Member Services at (617) 495-2008.
Remember - if you are eligible and wish to waive both the BCBS and SHF, you must complete two separate waiver applications.
NOTE: It is essential to keep a copy of your confirmation page. HUHS Member Services will not review any disputed charges after the deadlines without a print copy of your confirmation page. There are no exceptions to this policy.
