 The HUHS Medical Records/Health Information Services department processes your health documentation, immunization forms and requirements, and authorizes access to your online medical record.

**Hours of Operation for Medical Records:**  
Monday – Friday: 9 am – 12 pm and 1 pm – 5 pm (closed daily from 12 pm – 1 pm)

Immunization requirements for students are based on the [**Massachusetts Department of Public Health (MDPH) regulations for school entry**](https://www.mass.gov/info-details/school-immunizations), as well as additional institutional requirements where appropriate for our campus community.

Harvard follows the Commonwealth of Massachusetts immunization requirements for all students. In instances where national recommendations and Massachusetts law differ, Harvard will follow Massachusetts state law.
