On behalf of Harvard University Health Services (HUHS), welcome to Harvard! As a student, you must meet Harvard’s and Massachusetts’ immunization requirements. If you do not submit the required immunization records, you may be blocked from registering for classes. We understand that this process can feel overwhelming, so we’ve outlined the steps you need to take. Please follow the steps closely to ensure that you submit your immunization records in a timely fashion.

Your health is our primary concern. Please bring this letter and forms to your physician for careful review and completion. Note that when you return these forms, you are required to also submit the laboratory reports that verify your immunity to infection with measles, mumps, and rubella.
If you have a history of chicken pox infection (varicella), you are required to also submit the laboratory report that verifies your immunity to varicella. If you received the two-part varicella vaccine, you do not need to submit a blood test report verifying immunity to varicella.
If you completed the three-part hepatitis B vaccine series, you are required to also submit the laboratory report that verifies you have a quantitated titer of hepatitis B surface antibody (HBSAb). If the HBSAb is not consistent with immunity, have a single hepatitis B vaccine booster and repeat the quantitated titer after 30 days. Similarly, if one or more of the measles, mumps, or rubella serologies is not consistent with immunity, then have an MMR booster and repeat the appropriate quantitated titer(s) after 30 days.
You are required to have an updated dose of Tdap, the tetanus, diphtheria, acellular pertussis vaccine. There is a charge for this vaccine when it is administered at Harvard University Health Services (HUHS).
The preferred tuberculosis screening test is an interferon-gamma release assay (IGRA), such as Quantiferon Gold Plus or T-Spot. If an IGRA is not possible, you will need 2-part tuberculin skin testing (PPDs) unless latent tuberculosis has previously been diagnosed. The 2nd PPD must be at least 7 days after the 1st. If you have a positive IGRA or a PPD interpreted as consistent with latent tuberculosis, you are required to submit a radiologist’s report of a chest X-ray.
Meeting these requirements is an important first step before you matriculate at Harvard to protect your health and the health of your patients.

Immunization Requirements Overview
This information highlights your immunization requirements to be eligible to register for class.
General
- Rolling Compliance – Upon matriculation into your program, you will be placed on a registration hold until requirements have been satisfied.
- A registration hold means you will not be able to register for your classes if you have not met the immunization requirements.
- Some schools apply late fees for students who do not register by the school’s deadline.
Time Frame for Processing
- You are responsible for knowing the registration deadline for your program/school. Please plan accordingly.
- Immunization documents will need to be verified by HUHS. This may take 10 to 15 business days after you upload your documents.
- HUHS Health Information Services will contact you if you have additional unmet requirements or if your submitted documents do not meet the requirements.
- Students who submit documents within 10 business days of the registration deadline risk not being cleared in time to register.
How to Submit
- You can access the Patient Portal once you have matriculated and have received and claimed your Harvard Key.
- Contact your Registrar for Harvard Key questions.
- You must upload all vaccination documents directly to the Patient Portal. Do not email your documentation to us.
- Enter vaccination dates in the portal, in addition to uploading the supporting documentation.
Requirements for Uploads
- Documents must be in English.
- Documents must include your name, date of birth, date of vaccination, and name of vaccine given.
- The Immunization History Form must include the signature of your healthcare provider. The provider’s professional signature stamp at the bottom is required. Official hospital or office documentation completed by your provider can be submitted in lieu of the Immunization History Form.
Where to Get Your Immunizations
- You should receive your required immunizations before you arrive at Harvard.
- Many private health insurance plans cover the cost of immunizations.
- If you are unable to obtain these prior to your arrival on campus, you may arrange to get immunizations at various locations in the area.
- Please note that your health insurance plan might not pay for immunizations you receive, and you are responsible for the cost of the immunizations.

Please Note: Immunization verification can take a minimum of 10 – 15 business days to process and clear for registration.
Log into the HUHS Patient Portal to check the status of your submission
